    Abstract: 本計畫透過導入數位健康與人工智慧技術,建立高齡族群健康狀態之長期、居家化監測與風險預警機制,預期可促進衰弱綜合症及相關健康惡化狀態之早期辨識與個人化管理,進而降低衰弱所衍生之跌倒、失能、住院與照護依賴風險,提升高齡者之生活品質與自主生活能力,並減輕健保體系與長期照護系統之整體負擔。在產業與社會發展層面,本計畫結合臺灣在智慧型手機、穿戴式感測器與資訊通訊技術之既有優勢,發展具可擴展性與可重複部署性的數位健康系統架構,除可作為智慧長照與慢性病管理之技術基礎,亦有助於培育跨域整合之數位健康應用模式,促進國內智慧醫療與高齡科技相關產業之發展,並提升臺灣於國際數位健康領域之能見度與競爭力。
